Because they're the only compartment that can coordinate an entire immune response at once. In peacetime, immunity runs decentralized. Under threat, hierarchies appear — and myeloid cells are the control layer they form around: priming T cells, conditioning them, recruiting reinforcements, setting the cytokine milieu, seeding memory. Other compartments can turn a tumbler or two. Only myeloid cells turn them all at once.

Because a cure is a control problem, not a drug problem. Cancer is like a combination lock with five tumblers: neoantigen priming, T-cell conditioning, checkpoint release, cytokines and the TME, immune memory. Current therapies pick one at a time. The tumor jams all five — by seizing the layer that coordinates your immune response. Solid tumors converge on the same trick: seize the myeloid layer and the response goes dark. A coordinated system can't be beaten with uncoordinated drugs. All five tumblers have to turn at once, without the toxicity of forcing them. The tumor proved the operating system exists. By stealing it. IBOS steals it back — and makes it controllable.

Our risk sits in the engineering, not the biology. Most cancer drugs begin with an unproven molecule and hope it translates. Roughly one in eight immuno-oncology programs makes it from Phase 1 to approval — twice the oncology average.
